You may also speak with our clinic director to understand more about the services we provide, and determine if it is a right fit for you and your child. Moving forward, we request that you submit an Intake Questionnaire (click link below). Upon submission of the form, your child will be placed on our waitlist. The wait time can vary, depending on availability of therapists and your scheduling needs. Our office administrator will also advise you on obtaining a physician's referral, and payment options. Those choosing to self-pay will receive a Good Faith Estimate prior to the first appointment.

Intake Questionnaire Please complete the following questionnaire to assist us in determining services that your child may require.
RPTA is in network with the following insurances: *Sutter (both Sutter Select and Sutter Health Plan) If we are not in network with your insurance carrier, there are a few options: 1) You could ask your insurance carrier if there are other comparable local pediatric providers within your plan.
2) You could self-pay, and not go through the insurance system.
3) If you have a plan that reimburses you for outside providers, you can pay RPTA upfront, and we will provide you with a "Superbill," which is a receipt that you submit to your insurance company, according to your plan's guidelines for reimbursement.
4) We can seek a Single Case Agreement. In this case, the insurance company has to first authorize the service being requested , and then, secondly, negotiates a rate with RPTA. With a Single Case Agreement (sometimes called a Letter of Agreement), the family would only be responsible for their usual co-pay after the deductible has been met.
5) If your insurance plan does not allow Single Case Agreements, or we do not come to a contract agreement, they may authorize an Out-of-Network Exception, which only reimburses at their allowed in-network rates. In this case, we ask the family to sign a waiver, which says that the insurance company may pay a portion of our fees, and the family would be responsible for the remainder.
Children under the age of three may also be eligible for other funding sources.
